New patients are required to complete some brief paperwork. You can get started at home by printing the forms below and bringing the completed paperwork to your first visit with us.
After completing your paperwork, please call (203) 440-9686 to schedule your appointment with us or book online here: https://appointments.mychirotouch.com/?clinic=MC0309. You will meet with Dr. O’Donnell to discuss your health-related issues, physical abilities, medical history, and answer questions you have about treatment.
You will receive a thorough examination which may include X-rays to determine the best course of treatment for your specific situation.
Your first treatment will be scheduled with our receptionist and is usually within 2-3 days. Based on your initial examination and/or X-Ray findings, it can consist of spinal adjustments, physical therapy, and/or soft tissue therapy.
Most insurance providers cover chiropractic care; however, our staff will work with you and your insurance company to ensure you get the maximum benefits from your policy.
